- fractures (not fingers or toes)
- amputations
- loss of sight
- crush injuries (head, torso, internal organs)
- severe burns
- unconsciousness
- in hospital more than 24 hours
- over 7 day injury
- injuries to members of public or people not at work
- work related disease
- something that could cause injury

A
- elimination
- substitution
- safe working procedures
- training, instruction and supervision
- personal and respiratory protective equipment (PPE/RPE)
