initial:
analgesia
IV fluids
antibiotics

definitive:
<5mm - watchful waiting
<2cm - shockwave lithroscopy 
<2cm + pregnancy - flexible uteroscopy
>2cm or complex stones - percutaneous nephrolithotomy
