- Sudden, severe pain (10/10)
- Colicky in nature
- Loin to groin
- N/ V
- Sweating

Indications for surgical (procedural) Mx of stones:
A
- Failed conservative Mx
- Stone > 4/52
- Single kidney
- Reduced kidney Fx
- Septic
- Work - pilot, bus driver

What to do if septic w/ obstructing stone?
A
- D/W REG - surgical emergency
- Immediate decompression
- Sepsis 6 - IV ABx = IV Gent + IV Co-amox

Initial investigations for stones:
A
- Bloods - FBC, U&E
- Urine - dipstick, culture
- Imaging - CT KUB

Initial Mx - Medication:
A
- Analgesia - IV Paracetamol, PR Diclofenac, severe - Morphine
- Anti-emetics - Ondansetron, Cyclizine

Mx of distal stone < 5 mm in size:
A
- D/C home
- Analgesia
- Tamsulosin 400 mcg OD
- OP CT-KUB in 2/52
- Book Stone clinic in 4/52 (Hot Clinic)

Mx of proximal stone / > 5 mm / refractory pain:
A
- Admit
- Analgesia
- Book for theatre
- URS + LSF + stent (distal stone)
- De-obstruct with JJ stent & delayed URS + LSF (after 6/52)
- ESWL (proximal stones)
