- Think of therapeutic index (concentration range between efficacy and toxicity)
- As accumulation occurs this will push up plasma concentration and increase

Glomerulonephritis - penicillin, phenytoin
Nephritis – penicillin, NSAIDs, diuretics
Acute tubular necrosis - Aminoglycosides, Amphotericin and Ciclosporin
