Why is communication with the lab important?
It is part of the GDC standards and it will lead to the best outcome for the patients
What is the scope of practice for a Dental technician as by the GDC?
Dental technicians are registered dental professionals who make dental
devices to a prescription from a dentist or clinical dental technician.
They also repair dentures direct to members of the public without need for a prescription from a dentist
Are dentist countable for any faults in the medical device (denture)?
Dentists are allowed to refer(delegate) responsibility for a task but not accountability. therefore if something goes wrong the dentist would still be held accountable.
Dentists must only refer a task if they are confident of the capability and sufficient training carried out.
How are Lab prescriptions sent?
In person (the dentist can talk directly to the lab technician)
Over the phone
Or by email( where the dentist can send photographs)
These must all follow the data Protection act 2018 so if they include personal details of a patient it must be sent through a secure email (NHS email) and make sure that the technician has a secure email too)
What basic information should a Lab prescription involve?
1- Basic patient identifier (Name/DOB)
2- Date the lab card
3-Put your name on the card and address if in practice (& supervising clinician)
4-Date the next appointment
5-Indicate what you are making eg: F/F; -/P; lower soft splint
etc (pt may not want two P/P)
If its an immediate denture note this clearly and which teeth
are to be extracted (put a X on the teeth)
6-At EACH STAGE indicate that work going to the laboratory has
been disinfected
If we are prescribing for special trays what do we include?
1-Material to be used eg: PMMA (light cured acrylic)
2- Spacer
3-Tray handle and/or stops
4- Special instructions eg: horseshoe tray ( when pt gags)
what should a spacer be in a special tray?
usually 3mm for Alginate; exception close fitting tray in complete dentures for use with ZnO/Eugenol impression paste when ridge is resorbed
what should a prescription include for record blocks?
1-Do you need upper and/or lower
2- Base eg: wax; wire strengthener; shellac(in replica); Co/Cr etc
what should a prescription include for partial dentures?
1- Prescribe for 1ry cast to be surveyed & articulated
2- Indicate design clearly on card and 1ry cast
3-Indicate material of base eg: Co/Cr or acrylic and
4- materials for other elements eg: clasps
what should a prescription include at the try in stage?
1-Shade
2-Mould
3-Cusped/cuspless teeth
4- Setting
5- Individual requirements eg: 1mm diastema or no 7’s etc
what should a prescription include at the finish stage?
1-Postdams – where; how many
2- Relief areas – tori/overdentures
3- Soft lining?
4- Type of acrylic eg: high impact (if high risk of fractures) or heat cured
5- Special requirements eg: gum contouring (when pt has high smile line)
who is responsible for the statement of manufacture of medical devices?
It has what materials was used and where
Laboratory is responsible for producing it
Clinician responsible for offering it; note it
down in records; keep a copy
Quality Control Procedure (if pt has allergies we can look it up)
when do we not offer a statement of manufacture of medical devices?
non-medical devices eg:
gumshield
What is occlusion?
The static relationship between the incising or masticating surfaces of the maxillary or mandibular teeth or tooth
analogues
what is the Intercuspal Position (ICP)?
The complete intercuspation of the opposing teeth independent of condylar position, sometimes referred to as the best fit of the teeth regardless of the condylar position
