What is relining?

A

Straight forward substitution of layer of impression material within denture by a layer of new acrylic resin. This increases the thickness of upper denture . Impression material will change thickness
PVS lightbody will be minimal increase
Can affect compliance/ retention

What is rebasing?

Indication

A

Replacement of most of original acrylic base of denture. Used for UPPER denture as less likely to increase the thickness.
Deteriorated base/history of previous fracture
looseness of denture - unbalanced occlusion

A

Occlusion must be balanced.
Over/underextension corrected
Undercuts within impression surface eliminated
WARN PATIENT THAT THEY’LL HAVE TO BE WITHOUT DENTURE - Immediate denture

A

Distribute load more evenly and absorb impact - cushioning.
Improves masticatory performance and better comfort than hard relines.
Dimensionably stable/improved accuracy and fit

Treatment approaches for a flabby ridge

A

Surgical:
-Excision of fibrous tissue - Osseointegrated implants
Contraind - older patients with complex MH/probably too little bone for implants/removal of fibrous tissue will reduce extent and volume of denture bearing area

NS:

  • Mucodisplacive impression technique - compress flabby tissue to obtain max support
  • Mucostatic imoression technique for max retention (BEST OPTION)Mos

Detailed explaination of impression technique for a flabby ridge

A

-Primary imps - stock tray and low visc alginate (reduced tissue displacement) - mark flabby ridge on preliminary cast
-Special tray - area of flabby risge left uncovered. - low viscocity silicone, alginate, compound used for normal mucosa.
Reduce pressure by cutting perforations in tray at flabby tissue.
