Define GERD:
symptoms or mucosal damage produced by the abnormal reflux of gastric contents into the esophagus
Diagnosis criteria for GERD
heartburn two or more times a week
What are the causes of GERD?
lower esophageal sphincter relaxation and reduced resting pressure
What are the aggressive factors in GERD?
gastric acid, pepsin, bile acids, and pancreatic enzymes
What are the defensive mechanisms in GERD?
- Lower esophageal sphincter pressure

What are the typical symptoms of GERD?
- heartburn
- regurgitation
- belching
- dyspepsia
- epigastric pain & nausea
What are the extraesophageal symptoms?
- cough
- asthma
- laryngitis
- chest pain
- dental erosions
What are the alarm symptoms?
- dysphagia
- odynophagia
- unexplained weight loss
- bleeding
- anemia
- choking
- continual pain despite therapy
How is GERD most commonly diagnosed?
Symptoms (trial PPI and look for resolution)
When should a pt be referred for an endoscopy?
- age > 45
- alarm symptoms
- refractory to tx
- pre-op assessment
What are three major consequences of GERD?
- esophagitis
- development of strictures
- Barrett’s esophagus
What is Barrett’s esophagus?
A precancerous condition caused by prolonged GERD
