Realistic Medicine Flashcards
Define realistic medicine
Healthcare that focuses on true value to the patient.
Waste in healthcare should be assessed as interventions that don’t add value for patients e.g. avoiding unwarranted variation in clinical practice and resultant outcomes.
Guidelines developed for people with single diseases should not necessarily be extrapolated for patients with multiple conditions, this may result in over-treatment and over-complex medication regimes
What are the goals of realistic medicine
Build a personalised approach to care
Change our style to shared decision-making
Reduce unnecessary variation in practice and outcomes
Reduce harm and waste
Manage risk better
Become improvers and innovators
Describe personalised care (extrapolated)
Providing only the required information but all the required information
Doing so in a model the patient can understand
Helping the patient understand risk so they can make informed decisions
A survey found 3 main characteristics for a good doctor, what were they?
knowledge/qualifications
Good listener
Friendly/approachable
A survey found 3 things that patients felt made a good consultation with a doctor, what were they?
feeling listened to and not rushed
Clear communication
Resolution/diagnosis/outcome
Reduce unnecessary variation in practice and outcomes - what does this mean?
Honestly the tutorial were vague but…
The use of evidence based medicine and guidline in practice narrow down unneccessary variation
but
sometimes these interact poorly and cant be followed perfectly
Factors affecting of reducing harm and waste
Over diagnosis
Over treatment
How do we manage risk better
Safety netting
Use the MDT
How do we become improvers and innovators
on using data to support discussions and learning in clinical teams
Reflection
What 5 questions are patients encouraged to ask to ensure the right decisions for them
(this is 1 learning outcome)
is this test/treatment/procedure needed? What are the potential risks/benefits? What are the possible side effects? Are there simple/safer/alternative treatments? What would happen if I did nothing?
