Reticulocytes
Supra vital stain for RNA.
Methylene blue shows RNA meaning reticulocyte.
Indicative of anemia.
Show last 24 hrs of synthesis. Take 24 hrs to mature.
Normal 3%
Epo synthesis
Renal Cortex. Interstitial cells in peri tubular capillary.
Can be synthesized in renal cell carcinoma or hepatocellular carcinoma.
Increased erythropoiesis causes medullary widening (hair on end skull.)

B, T, NK cells
Polychromatic RBC
Younger than reticulocyte. Whole cell appears blue on smear.
2-3 days for maturation.
Extra medullary hepatopoesis
Liver and spleen
Hepatosplenomegally.
Fetus in yolk sac and spleen
Anemia symptoms
Fatigue. Dyspnea. Dizziness.
Pallor of skin and pulmonary murmur are severe.
Microcytic anemia
MCV < 80 Decreased corpuscular hemoglobin Iron deficiency ( with increased RBC width ) Anemia of Chronic disease Thalassemia Sideroblastic
Normocytic anemia
MCV 80 - 100
Normocytic with low reticulocyte count
Early stage blood loss, iron def, chronic disease
Renal disease
Malignancy
Aplastic anemia
Normocytic with reticulocyte above 3%
Intrinsic: sickle cell, hereditary, G6PD, pyruvate kinase
Extrinsic: blood loss >1wk, hemolytic anemia
Malaria
Megaloblastic anemia
MCV > 100
Folate or B12 deficiency
Also see hyper segmented neutrophils.
Glutathione GSH
H2O2 to water. Also acetone nephron free radicals.
Synthesized via pentose phosphate pathway
Methemoglobin reductase pathway
Converts fe3+ which can’t bind o2 to Fe2+ which can.
Synthesize 2,3 BPG
Right shift in binding curve. Hold on tighter to o2.
Senescent RBCs
Last 110-120 days
Phagocytosed by splenic macrophages. Turn heme into unconjugated bilirubin.
