Aplastic crisis

A

Acute drop in Hb & low reticulocyte count (<1%) without splenomegaly. Means sudden stop in RBC production (erythropoiesis). Severe anemia <6 g/dl. Systolic murmur from hyper dynamic flow (less blood will move faster). Anemia leads to high output cardiovascular failure. WBCs & platelets normal. Leads to hydrops fetalis & ascites.
MCC: Parvo B19 affecting RBCs & destroys RBC precursors.
Tx: Blood transfusion.
(The low reticulocyte count is the key).

Normal reticulocyte count = 1%.

Juvenile myoclonic epilepsy

A

Teens, myoclonic jerks first hours of awakening. Myoclonus worse if sleep deprivation. Confusion = seizure.
