most commonly affects trunk with hypopigmented, pink or brown patches
management: topical antifungal (e.g. ketoconazole shampoo)
if failure to respond, give oral itraconazole

1) oral steroids
2) ciclosporin or infliximab in difficult cases

1) permethrin cream
2) malathion cream
advise patient that pruritis persists 4-6w following eradication
avoid close physical contact
wash all sheets bedding etc
