Radiology of Arthritis & Related Conditions Flashcards
4 Major Types of Arthritis
- Non-inflammatory (degenerative)
- Inflammatory
- Metabolic
- Infectious / Septic
4 Types of Degenerative / Non-Inflammatory Arthritis
- DJD / DDD
- DISH
- Neurotrophic
- SCM (synovial chondrometaplasia)
7 Types of Inflammatory Arthritis
- Psoriatic
- Enteric
- AS
- Reiters
- OCI (osteitis condensens ilii)
- OP (osteitis pubis)
- RA
3 Types of Metabolic Arthritis
- Gout
- HADD
- CPPD
Is the IVD a synovial joint?
no, fibrocartilage
Where does degeneration begin in the IVD?
AF fibers
Clinical features of DJD
- Common sites: weight bearing joints and fingers
- All lab tests are negative
Radiographic Hallmarks of DJD
- non-uniform joint space narrowing
- osteophytes
- sclerosis
A radiographic sign of DDD in the disc itself. Can be a normal variant in synovial joints.
Vacuum disc sign (aka = phenomena of Knuttsen)
AKA for kissing spine syndrome
Baastrup’s Syndrome (interspinous OA)
An old person’s diseased hip is called ______
Malum Coxae Senilis
Latin word for diseased or evil
Malum
Radiographic signs of an old person’s diseased hip
- non-uniform joint space loss
- osteophytes
- sclerosis
- bone cysts
- flat femoral head at its superior position
DJD of the knee - MC feature and location
Hypertrophy of the tibial spine
Medial knee MC
DJD of Ankle & Tarsal Jts
MC feature
Degenerative calcaneal spurs
DJD of foot
MC joint involved
Common features
MTP of digit 1 (hallux)
Loss of ROM causes hallux rigidis
DJD causes a bunion and hallux valgus deformity
2 AKAs for DISH
- Forestier’s disease
- hyperostosis ankylosing senilis (He “has” DISH)
What does DISH stand for?
Diffuse
Idiopathic
Skeletal
Hyperostosis
Ligament usually affected by DISH?
ALL
Clinical features of DISH?
- Males > females
- Age > 50
- associated with Diabetes M
- No lab findings
MC location for DISH?
T spine
If DISH occurs in C-spine, possible complications?
- dysphagia
- ossification of PLL leading to spinal stenosis = more common in Asians
Radiographic Criteria for DISH are:
- 3 consecutive spinal disc levels (4 vertebral bodies)
- Non-marginal spondylophytes
- Non-uniform spondylophytes
- preservation of disc heights of the bridged sections
- No ankylosis
A radiographic “sign” of DISH
Dripping Candle Wax sign
AKA for Neurotrophic Arthropathy
Charcot’s joints
Disease most commonly associated with Neurotrophic Arthropathy?
Diabetes M. (52%) followed by trauma (48%)
MC location for neurotrophic arthropathy?
feet
“Signs” of Neurotrophic Arthropathy
Bag of bones
Tumbling block vertebra (lumbar spine)
Licked Candy stick (shoulder, hip, & foot articulated ends)
MC location for Charcot’s
Lumbar spine (total disorganization)
AKA for synovial chondrometaplasia
Synovial Osteochondromatosis (joint mice)
Name the disease (C-spine)

DISH
note OPLL as well as ALL
Name the disease

Erosive DJD
name the disease (L spine)

DISH
name the disease (C spine)

DISH
Name the disease (T-spine)

DISH
Radiographic signs of Psoriatic Arthitis in the spine
Syndesmophytes:
1) Assymetric
2) Non-marginal
3) Unilateral
4) Broad-based
Note: exceptions are made for C-spine in 1 & 2 above.
It’s all about the syndesmophytes
Malar Erythema from sun exposure
Reversible deformities of fingers and ulna
Systemic Lupus Erythematosus
SLE
Ulnar deviation of fingers
Fibular deviation of toes
No joint destruction
Joint spaces maintained
reversible deviations
Follows a Strep infection
Jaccoud’s Arthritis
name the disease of this young boy

Scleroderma
distal tufts reabsorbed
“Square-a-derma”
name the disease
Scleroderma
Triad of digital clubbing, systemic arthritis, periostitis PLUS a major visceral disease (usually bronchogenic carcinoma)
Hypertrophic Osteo-Arthropathy (HOA)
The AKA for Hypertrophic OsteoArthritis (HOA) is:
Hypertrophic PULMONARY Osteoarthropathy
Pulmonary because of association to bronchogenic carcinomas
