Subluxation of the C1-C2 joint
How does someone get an atlantoaxial subluxation
It is typically traumatic or congenital
What are the special populations at risk for atlantoaxial subluxation?
Rheumatoid arthritis
Down’s syndrome
Marfan’s syndrome
What are indications to order an MRI deemed appropriate by ACR?
New or increasing cervical radiculopathy
Suspicion of infection with new or increasing neck pain or radiculopathy
Known malignancy or history of malignancy and new/increasing symptoms
Chronic neck pain and radiographs indicate degenerative changes
When would we order an MRI?
Presence of worsening myotomal weakness, altered DTRs, dermatomal pattern of pain/paresthesia
Neck pain greater than 6 weeks and/or meet criteria for presence of radiculopathy
Chronic neck pain (greater than 3 months) and over 50 years of age
Signs and symptoms consistent with myelopathy/UMN signs
What is the utility of a sagittal cut MRI?
To see alignment
Central cord assessment
IVD fluidity/health
Joint spacing
Vertebral body congruency
What is the utility of an axial view MRI?
Foraminal narrowing
IVD annular ring assessment
