What is a Boutonniere deformity?
rupture of the terminal portion of the extensor hood which holds the lateral bands in place
What is a Swan neck deformity?
MCP and PIP hyperextension due to intrinsic contractures and DIP flexion due to contracture of the deep finger flexor muscles
Why does ulnar deviation of the fingers occur?
Weakening of the ECU, ulnar and radial collateral ligaments which causes the wrist to deviate radially and increases torque on ulnar finger flexors
Early RA can be confused with which disease?
de Quervain’s tenosynovitis - inflammation of the EPB and APL tendons due to tendon sheath thickening. Use Finkelstein’s test.
How do carpal bones rotate in RA?
Proximal row = volarly
Distal row = dorsally
Pseudobenediction sign is due to?
Extensor tendons of the 4th and 5th digits rupturing due to contact with the elevated ulnar styloid
Hammer toe is?
hyperextension of MTP/DIP and flexion of PIP
Claw toe is?
hyperextension of MTP and flexion of PIP/DIP
What cervical spine issue may arise in RA?
AA joint subluxation due to tenosynovitis of the transverse ligament of C1
Discuss rheumatoid nodules
in 50% of RA patients, can form in subcutaneous layer, bursae or tendon sheaths, usually on extensor surface of forearm, methotrexate may worsen, seen in RA and GOUT
Caplan’s syndrome
intrapulmonary nodules, RF+, assoc with RA and pneumoconiosis, is a granulomatous response to silica dust
