2 Confusion
The patient does not know what time it is or where they are at.
3 Delirium / Disorientation
The patient has Sluggish Speech.
4 Lethargy
The patient arouses with stimulation.
Ex. The nurse must tell “HEY WAKE UP!”
5 Obtundation
The patient only responds only to vigorous and repeated stimulation.
Ex. The nurse must yell at and shake the patient vigorously to arouse them.
6 Stupor
The patient has no motor or verbal response to noxious stimuli.
7 Coma
The patient has a permanent loss of function of the cerebral cortex of the brain.
8 Persistent Vegetative State
The patient is Alert and Oriented
