assessment overview
a sequence of actions that helps determine what is wrong, and thus helps provide safe and appropiate first aid
what is the assessment format?
- perform a scene size up
- perform a primary assessment
- obtain the person’s SAMPLE history
- perform a secondary assessment
- perform a reassessment
what does the scene size up entail?
determines the safety of the scene, the cause of injury or nature of illnss, and the number of people involved
what does the primary assessment entail?
checking the person’s responsiveness, checking for breathing, and checking for circulation and severe external bleeding
what are the 4 questions you ask if a person if responsive to evalute their memory?
person - what is your name?
place - do you know where you are?
time - what is the month and year?
event - what happened?
what do you do for an unresponsive, motionless person?
R- responsive? check responsiveness by gently tapping the person’s shoulder and shouting “are you ok?”
A - activte the EMS! if there is no response, call 911!
B - breathing? position the person on their back on a flat, firm surface. check for no breathing or only gasping. if the person is breathing, place them in the recovery position and check for severe bleeding, if not give CPR
what to do to control bleeding?
place a sterile dressing over the wound and apply direct pressure with a hand or a pressure bandage control the bleeding
the secondary assessment
where you will focus on the chief complaint and immediately treat any life-threatening conditions
what does DOTS stand for
D - deformity: the shape of the body part is abnormal
O - open wounds: the skin is broken and bleeding is present
T - tenderness: the person reports sensitivity, discomfort, or pain. press on areas to check for tenderness
S - swelling: the area looks larger that usual. swelling is caused by extra fluid in the tissue.
reassessment
regularly recheck the person who has a serious injury or illness such as breathing difficulties or major blood loss, or who has experienced a significant cause of injury.
what are two special considerations?
- exposing the injury
- avoid contact during a physical exam
what is the capillary refill test?
- gently press a fingernail or toenail for a few seconds and then release pressure
- when released, the blanched skin should reutrn to normal color within 2 seconds
- if not there is poor circulation present
