Involves some degree of damage or injury associated with the operation of an aircraft

Aircraft Accident
Aircraft damage is not repairable, or is damaged but not repairable. Aircraft is missing or wreckage is inaccessible

The aircraft is destroyed
There were multiple fatalities
The was one fatality and the aircraft sustained substantial damage
Major Accident
Causing one or more fatalities to occupants of the a/c
A single fatality without substantial damage
At least one serious injury and aircraft substantially damaged
Fatal Accident
Aircraft sustained minor damage
Minor Accident
Accident that affects or could affect the safety of the operation
Aircraft Incident
Injury which results in death within 30 days of the accident
Fatal injury
-2 days hospitalization up to 7 days after the accident

Serious Injury
Less than 2 days up to 7 days after the accident
Minor Injury
Aircraft is not repairable, if repairable, the cost is more than 50% of the cost of the a/c when it was new

Damage of failure that adversely affect the structural strength, performance, or flight characteristics of the aircraft, and which would normally require major repair or replacement of the affected component
Substantial
Damage that neither destroy the aircraft nor causes substantial damage

Explanatory causes
Most important aspect of the investigation
To prevent the accident from happening again and correct the deficiencies identified in the process

Organizations may have some authority or reponsibility
Key players
It does set the ground rules on how aircraft accidents involving more than one country (state) will be investigated
International Civil Aviation Organization
Person or group charged with conducting the investigation
Investigator in charge
Independent board charged with investigating all civil (and some public) aircraft accidents in the United States
National Transportation Safety Board
Government agency responsible for safety in the country, not investigation. Participate in investigation or may investigate if delegated to them
Civil Aviation Authority

Right and responsibility to conduct investigations of accident occurring in its territory. Can be state of registry or state of the operator
State of occurrence
When the accident occurs outside the territory of any state, the state of registry assumes the rights and responsibilities of the state of occurrence
Accident in international waters
When notified of an accident involving one of its aircraft, they are obliged to furnish the state of occurrence with any available information regarding the aircraft and flight crew
