6. How did the assault take place? Did they use any instruments?
7. Was there physical abuse?
8. Do you currently have any pain? Where? (OLDCARTS)
9. Any bleeding or bruising?
10. Did you lose consciousness?
11. Was there sexual abuse?
12. Did they use condoms?
13. Did ejaculation occur?
14. What types of intercourse were performed?
15. Were any foreign objects used?
16. LMP?
17. Are you currently using contraceptives?
18. Has there been any vaginal bleeding?
19. Any other trauma or issues we should address?
20. Full remaining medical Hx (PMHx, PSHx, etc.)
