Tell us about a time that you lost SA, and what you did to regain it
S - Shortly after starting instructing in Melb I was conducting a VFR Nav exercise north of the CTA
T - Student commenced a climb just the other side of the control step but had no techinque - not trimming, entering climb without clear the airspace or applying full pwr, not rudder. I became so engrossed in the teaching part of the lesson trying to correct his actions that I allowed the A/C to clip CTA step
A - ATC did general broad cast and I realised it was me they were trying to contact so I got on the radio, they informed me what I had done but that I was now outside the step
I apologised and filled in a report when a got back on the ground for the company - no action was taken or recorded but I learnt a valuable lesson about the priorities of instructing.
First and foremost is the safety and professional conduct of the flight and then the teaching, at times the teaching has to stop inorder to manage the flight
Tell us when you went beyond for your company.
- Ready to leave after long shift, found out there was no one to supervise and that there were still solo students out flying so I cancelled my plans to stay around to ensure they got back

** - Don’t really do the massive things but when I see a hole or need I try to address it or bring it to the attention of someone who can fix it

o Had Annual leave booked and approved from mid December through to the New Year to spend time with family over Xmas
o Company very short staffed over this time due to others taking leave and this was going to have a negative effect on productivity and student progress (international students) - A
o I elected to change my holiday until the new year to help with this problem
o Worked through only taking our non trade days off until other staff member s returned form holidays - O
o Still got to spend time with the family while my boys where on holiday and

Leaving Darwin and returning to Melb after family could move up
Having to leave a flying job when the company became irratic with my pay and taking 6 months to get another flying postion - Av College, company went back shortly after

What was your hardest day in Aviation?

Last flight of the day – unanticipated
Long day hot day started around 7am, 3rd flight for the day and I was tired, landed from previous flight and told the Chief pilot wasn’t around and I needed to go to Palm to unload freight, daily company run
I was flying with a person I considered to be a good pilot, (he had been with the company 6mnts ish, quickly promoted by CP to the Twin and had about 50 hrs on type.
He was recent and I had never had any issues when I had flown with him before including that morning.

Flight over was without incident we unloaded freight and waited for the pax
On the flight back I was complacent and didn’t properly monitor the other pilot’s performance and as a result let down both him and the paxs
Saw him enrichen the mixture and turn on fuel pumps and presumed he had completed his pre-landing checks
During approach noticed that he was off profile, commented and he acknowledged and took actions to correct
In the flare things seemed off – I don’t know exactly why but as I called for a Go Around he was already powering up
I assumed we had had a gear failure and whilst he flew I took over radios
Received to response on Coms 1 so switched to comms 2 – got the twr and reported in the Go Around
Let the tower know that we had a gear problem and climbed away to rectify
PF slowed the plane to emergency gear speed and selected gear down,
Gear came down without issue – 3 green
Elected to land immediately as unsure of damage to the A/C

Landed without further incident
Completed S&P check with CP
Learnt – if the picture isn’t correct – there is a reason – find it
If you are in the front you have a job to do, It doesn’t matter how experienced a pilot is we can all make mistakes
Now check the gear routinely several times between selecting it down and touch down whether I’m PF or not

Tell us about a time you shot yourself
15th July 2024
S - Flying with a student early in their C90 training when returning back MB @ night, tracking back to MB Wx radar showed WX had turned much worse than forecast, altered my approach to RNP to aviod Wx over the bay but could really see the Wx behind the cell
T - Conducted first IAP, when crossing the coast flew into very heavy rain, strong wind and turbulent to the point that the A/P could keep up so had to hand fly, I thought i had the RWY light in sight but quickly realised the orienation wasn’t right given the wind so conducted MAP and returned to MMBSA to hold
2nd approach didn’t get visual until durng the missed approach - committed to MAP so returned to the hold to consider my options - listened to the ATIS at other aerodromes and determined that YMEN would be my best option did some additional fuel calcs and set a limit of 800 Ibs prior to buggin out this allowed some hoding and 1 more attempt, monitored the AWIS as TWR closed and Wx kept getting worse - belows mins so made a command decision and got ATC clearance to conduct EN ILS - landed without incident and got a lift back to MB fro myself and the student, another instructor picked up the plane the next day
Learnt not to trust BoM and always have a backup plan in place

How will you handle back of the clock flying
Worked back of the clock before,
I know it can be hard to balance and adjust to especially initially as you adjust to the new schedule
Lucky that I can be flexible with my sleeping patterns and naturally tend to be a night owl
