What is the purpose of DQI?
To encourage continuous improvement across a broad range of disease management processes
DQI success is achieved by:
Managing individual patients first and allowing scores to follow
Knowing each team member’s role in improving DQI scores
Using the CQI process to help identify trends and make improvements

Encourage placement of permanent access (AVF/AVG)
