HPI questions for PV
A
pain in arms and legs? pain or cramping in legs with walking or at rest? how far can you walk without stopping? is the pain better with rest? coldness, nubness, pallor in legs or feet? hair loss in lower extremitites? ulcers? discoloration of skin? redness? unilateral/bilateral edema? fingertips change color in the cold? what color do they change? UE or LE skin blisters? dry scaly skin on UE or LE H/A dizzy or light headed? gait disturbances? vision issues? facial droop abdominal pain? abnormal pulsations in abdomen? abnormal lumps in abdomen?

A
HTN DM hyperlipidemia CAD MI angina CVA trauma hypercoagulable states high levels of homocysteine DVT/PE previous h/o vascular surgery

PAD manifestations
A
diminished pulses smooth shiny dry skin loss of hair on LE no edema round, regular shaped painful ulcers on distal foot, toes, webs of toes dependent rubor pallor or pain when legs are elevated intermittent claudication brittle thick nails

A
sudden pain pulselessness pallor paresthesias paralysis

chronic s/s of PAD
A
intermittent claudication rest pain tissue loss --peripheral = ulcer --cerebral = TIA/CVA --visceral = mesenteric ischemia/ bowel infarction

risk factors for PVD
A
tobacco use alcohol use cocain use PAD/CAD/CVD/DVT/PE - family hx BMI>30 age >50yo recent long distance travel prolonged inactivity pregnancy or post partum malignancy

PVD - venous manifestations
A
normal pulses brown patches of skin discoloration on LE dependent edema irregular shaped, painless ulcers on LE and ankles dependent caynosis and pain pain relieved with legs elevated no intermittent claudication normal nails
