Pulmonic Valve Flashcards
1
Q
Pulmonic Valve views and anatomy
A
- Most anterior valve —> hard to see with TEE — anterior, rightward, and perpendicular to the AV
- Pathology usually congenital therefore not typically seen in adults
- Left cusp is closest to the AV, anterior cusp furthest from AV — ME RV inflow-outflow shows AV in SAX = PV in LAX with anterior cusp in far field and R/L cusp closest to AV
2
Q
Pulmonic Insufficiency
A
Mild PI = normal variant Significant PI usually congenital May be acquired (carcinoid) but rare Infective endocarditis least likely affects PV Etiology: •carcinoid •endocarditis •pulmonary hypertension •myxomatous disease •thorax radiation
3
Q
Pulmonic Insufficiency Grading
A
Similar to AI…
Jet area Jet width Holodiastolic flow reversal in main PA •early velocity —> PA mean pressure •late velocity —> PA diastolic pressure
4
Q
Pulmonic Stenosis
A
Usually congenital
Peak / Mean gradient with CWD
Subtract the max PS gradient from the RVSP to obtain the PASP •CWD of TR jet —> RVSP •RAP by central line CVP •RVSP - PASP = PS pressure grad ... PASP = RVSP - PS pressure grad
