The DLCO narrows the differential diagnosis of restrictive lung disease.
A low DLCO combined with reduced lung volumes suggests interstitial lung disease. A normal DLCO associated with low lung volumes suggests an extrapulmonary cause of the restriction, (pleural effusion, pleural thickening, neuromuscular weakness, or kyphoscoliosis.)
