what are the symptoms of pulmonary fibrosis?
A
- dry cough
- SOB on exertion
- malaise
- low weight
- arthralgia

what are the examination signs of pul fibrosis?
A
- clubbing
- fine crepitations / crackles on auscultation
- cyanosis

what are the complications of pul fibrosis?
A
- resp failure
- increased risk of lung ca

What are the invx for pul fibrosis?
A
Bloods:
- ABG (reduced PaO2, if severe then increased PaCO2)
- raised CRP
- Rheumatoid factor ( +ve 10%)
- FBC - raised lymphocytes (good prognosis), (if raised neuts/eosinpophils then poor prognosis)
Chest xray shows:
- decreased lung volume
- bilateral lower zone reticulonodular shadows
- honey comb lung
CT
Spirometry -restrictive lung disease
Lung biopsy

What does the ABG of fibrosis show?
A
low PaO2 (if severe fibrosis then PaCO2 will be raised)

What does a raised neutrophil / eosinophil count indicate in pulmonary fibrosis?
A
poor prognosis

What does a raised lymphocyte count indicate in pulmonary fibrosis?
A
good prognosis

opiates
lung transplant
(dont give high dose steroids)
