  • Used to inform people outside of the hospital of the patient’s wishes (ex: nursing home staff, EMTs, etc)
  • The phsyician has already “signed off” on the orders

Who should have one?

  • Write one with your patient if you would not be surprised if they died in the next year

  • The default surrogate can only refuse life support if the patient has a qualifying condition
