What is the pathogenesis of PUD?
Imbalance between the protective factors, and the damaging factors, a.k.a. defensive factors and aggressive factors
What is the primary pathways to stimulate gastric acid secretion?
Neurocrine pathway ->acetylcholine
Endocrine pathway ->Gastrin
Paracrine pathway ->histamine
Where does the parietal cells located?
In the wall of the midsection of the oxyntic gland
What does the oxyntic gland secrete
Gastric mucosa
What is the aggressive factors causing PUD
Acid
Pepsin
H. pylori
NSAIDS
What is the defensive factors causing PUD?
Prostaglandins
Mucus gel layer
HCO3
What is the complication of PUD?
Hemorrhage
Blood vessels damage
Occult blood and tarry stools
Perforation
Erode through the entire wall
Peritonitis
Narrowing or obstruction
Swelling and scarring cause obstruction ->repeated vomiting
What is the risk factors that can cause PUD
1- Lifestyle (smoke, acidic drinks, medication, caffeine)
2- H. pylori infection (90% have it, passed from person to person)
3- Age (duodenal 30-50, gastric over 60)
4- Gender ( duodenal older women)
5- Genetic factors (family history)
6- Disease (Zollinger Ellison syndrome)
7- Other factor (stress can worsen not cause)
What is the symptoms of PUD?
Burning epigastric pain
Pain relived with food
Nausea, vomiting, anorexia, a weight loss
Nocturnal pain
Aims for ulcer treatment
Heal
Relieve pain
No relapse
No complication
Major causes of PUD
Infection of H. pylori
Increase HCl
Inadequate mucosal defense
