False impression of a Ptosis may be:
•dermatochalasis
•lack of lid support from globe
•ipsilateral hypotropia
•contalateral lid retraction
•brow ptosis
Margin reflex distance
•Distance between the upper lid margin and corneal reflection
• normal is 4-5mm
Palpebral fissure height
•Distance between the upper and lower lid margins
Levator function test
•place thumb firmly against Px brow
•Px looks up and excursion is measured
• normal >15mm
Upper lid crease measure
•distance between lid margin and lid crease in downgaze
Pretarsal show
•distance between lid margin and skin fold while Px looks forward
Simple congenital ptosis
•Failure of neuronal migration
•failure of muscular development
Marcus Gunn jaw winking syndrome
A branch of CN V is misdirected to the lavator muscle
Marcus Gunn jaw winking syndrome treatment
Mild - unilateral levator advancement
Moderate - levator disinsertion
Bilateral levator disinsertion
IIICN palsy causes
Vasculopathic - diabetes/hypertension/atherosclerosis
Compressive - aneurysm/neoplasm
