INDEPENDENT
I
Patient is able to perform 100% of an activity independently, in a consistently safe manner
STANDBY ASSISTANCE
SBA
PTA supervises the patient’s independent work by standing by and being ready to give light assistance if needed. No hands on the patient, just ready. May need to give verbal cues.
CONTACT GUARDING
CGA
PTA supervises the patient’s independent work by continuously guiding and guarding the patient, with hands on the patient, due to perceived risk of falls or to provide tactile input.
MINIMAL ASSISTANCE
MIN A
Patient performs approximately 75% or more of the task, but requires assistance by the PTA for 25% of the total work. Assistance may be needed for balance, to move an extremity or assistive device, to lift the weight of the body in standing, etc.

MOD A
Patient performs approximately 50% - 74% of the task, but requires assistance for the other 50%.

MAXIMUM ASSISTANCE
MAX A
Patient has difficulty supporting his/her weight, but contributes approximately 25% - 49% of the total work. The PTA assists, doing approximately 75% of the task.
