Controlled arrhythmia

Uncontrolled arrhythmia
Tx in more controlled environment required
Controlled hypertension
Caution w/ epi and stress
- additional caution w/ drug interactions
CHF
Tx as normal
Stroke
Tx as normal
Currently at risk of a stroke
Caution with epi
Controlled diabetes
Tx as normal
Uncontrolled CHF
Reduce stress
Uncontrolled diabetes
Monitor for problems
Severe hypertension
Limit epi, maybe tx in more stable location?
Uncontrolled hyperthyroidism
Means the pt doesn’t know they have it
- caution epi and stress
Asthma
Tx as normal
Pheochromocytoma
Rare tumor
- defer tx if not treated
- avoid epi and stress
Renal disease
Tx as normal
- always treat day after dialysis
Liver diseases
Tx as normal
Pregnancy
Tx as normal
- avoid 1st and 3rd trimester if possible
Coagulopathy
Alter pathway of epi
Methemoglobinemia
Avoid prilocaine!
- lidocaine should be ok
COPD
May need to keep chair time shorter
Cocaine
Delay for 24 hrs
- caution with epi
Malignant hyperthermia
Problems with ester drugs (BUT WE DONT GIVE ANYWAYS)
- give amides
Atypical plasma pseudocholinesterase
Problems with esters
- give amides
Stable Angina
