Cognitive behaviour therapy(CBT)
Treating mental disorder based on cognitive and behavioural techniques
Aim to deal with thinking such as challenging negative thoughts and includes behavioural techniques
Family therapy
Psychological therapy with all or some members of a family with the aim of improving the communications within and reducing stress of living as a family
Cognitive behavioural therapy
Treat people with SZ. 5-20 sessions. Groups or 1:1
Helps clients make sense of how they irrational cognitions impact their feeling and behaviours
Therapist can explain that the voice isn’t demonic it’s a malfunction in their brain
Doesn’t eliminate the problem but helps cope with it
Normalise it -hearing voices and is an extension of reality
Reality testing -Examine the likelihood their irrational thoughts are true
Case example-Douglas Turkington
Client: the mafia observing me to decide how to kill me
Therapist: you are obviously very frightened… there must be a good reason for this
Client : do you think it’s the mafia?
Therapist : it’s possible but there could be other explanations. How do you know it’s the mafia?
Strengths of CBT-evidence for effectiveness
Jauhar-34 studies of CBT with SZ. Small but significant effect on positive and negative symptoms
Maria Pontillo -reductions in frequency and severity of auditory hallucinations
CBT is recommended for schizophrenia
Limitation of CBT-quality of evidence
Wide range of techniques and symptoms included-vary from case to case.
Thomas -overall benefits conceal variety of effect-hard to say how effective it will be for a particular person
Evaluation Does CBT cure?
CBT improves quality of life but doesn’t actually cure them
Studies report significant reductions of symptoms suggesting it does more than just enhance coping
Family therapy
Double bind-fears doing the wrong thing, mixed messages, mother withdrawals love, disordered thinking and delusions
Schizophrenogenic mother -cold, rejecting, controlling, tense. Causes distrust and delusions
Family therapy aims to improve quality of communication reduce negative emotions and encourage members to form a therapeutic alliance
Allows family to find a balance between caring for schizophrenic family member and maintaining own life
Model of practice-Frank Burbach
Share info and provide emotional support, identify resources that family members can/can’t offer, encourage mutual understanding and a safe space, identify unhelpful patterns, skill training, relapse prevention planning, maintenance for the future
Strengths of family therapy
McFarlane-consistently effective treatment-relapse rate reduced by 50 to 60%. Recommended
National Institute for health and care excellence -recommend for all people diagnosed with SZ
Helps with early and well established schizophrenia and benefits the whole family
Lobban and barrowclough-strengthening function of whole family lessons negative impact on other members and strengthens ability of the family to support the member with schizophrenia
