history: questions to ask about suicide ideation/self harm/homicidal ideation
- are you feeling safe?
- any thoughts of hurting yourself?
- any suicidal thoughts?
- any feelings that you may want to hurt others?
- have you ever felt like you need to defend yourself?
questions for general symptoms to ask about on psych visit?
- mood: how has your mood been?
- sleep: how has your sleep been? is that normal for you?
- energy: how has your energy been?
- concentration: how has your concentration been? are you able to focus? are you easily distractible? is that normal for you?
- appetite: how has your appetite been?
- perception: sometimes people can see or hear things that others cannot, are you experiencing any of that? ask them to describe
- risk: are you feeling safe? any thoughts of SI/SH/HI?

what does a form 1/42 get you?
form 1: 72 h hold for a psychiatric eval
form 42: notice to the patient of form 1 status
what does a form 3/30 get you?
form 3: 14d involuntary admit into hospital (including the day of filing)
form 30: notification of form 3 to the patient
what does a form 4/30 get you?
form 4: renewal of involuntary admit
- first form 4: an additional 1M
- second form 4: an additional 2M
- third form 4: an additional 3M
form 30: notification of involuntary admit
what does a form 33 get you?
it is used to certify incapable - can be certified incapable for 3 different things
- consenting to medical treatment
- taking care of their property and finances
- ability to share PHI
what does a form 50 get you?
to confirm that the patient has received rights advice; usually after a form 33 so that they may content at a consent and capacity board
