Pseudogout Flashcards
What is the diagnosis from this xray?
CPPD
What will polarised light microscopy of the synovial fluid from a joint with pseudogout show?
- weakly positive birefringent
- rhomboid shaped crystals
- of crystal-calcium pyrophosphate
What is chondrocalcinosis?
soft tissue calcium deposition
What are the 3 types of pseudogout?
- sporadic (irregular/unknown)
- heredity
- secondary
What is pseudogout also known as?
Calcium pyrophosphate dihydrate disease (CPPD)
What are the clinical features of acute synovitis?
- pain
- swelling
- stiffness
- occasionally fever
What 4 metabolic diseases predispose to calcium pyrophosphate dihydrate disease?
- hypothyroidism
- hyperparathyroidism
- acromegaly (overproduction of the growth hormone)
- gout
Where is chondrocalcinosis most commonly seen on x ray?
- in the menisci of the knee
- triangular cartilage of the wrist
- symphysis pubis
What will an X-ray of a joint with psuedogout look like?
white lines of chondrocalcinosis
What is calcium pyrophosphate dihydrate disease?
an arthropathy associated with the deposition of CPPD crystals
What are the clinical features of chronic pyrophosphate arthropathy?
- gradual onset of pain
- swelling
- loss of joint function
What causes the autosomal dominant variant of CPPD?
- germline mutations in the pyrophosphate transport chain
- resulting in crystal deposition
What are the two main clinical presentations of CPPD disease?
- acute synovitis
- chronic pyrophosphate arthroplasty
How can pseudogout be treated?
- analgesia, NSAIDS
- joint aspiration, intra-articular steroids
- cholchicine
Is gout or pseudogout more common?
gout
