Primary prevention - 20mg ON
Established CVD - 80mg ON
HTN - First Line Drugs
<55 - ACEI
>55 - CCB
Afrocaribean - CCB
Diabetic - ACEI
Folic acid doses
Low risk - 400mcg
High risk - 5mg
Bisphosphonates - patient advice
Take 30 mins before food or other drugs
Sit upright for 30 mins
Take with plenty of water
Gentamycin - interpretation of levels
Peak level high/low - change dose
Trough level high/low - change frequency
CYP Inducers
Phenytoin Carbamazepine Barbiturates Rifampacin Alcohol (chronic) Sulphonyureas (gliclazide)
CYP Inhibitors
Grapefruit Omeprazole Disulfiram EtOH (acute) Valproate Isoniazid Ciprfloxacin + Cimetidine Erythromycin (and other macrolides) Sulphonamides
Lithium - monitoring
Before starting: BMI, ECG, U&Es, FBC
6/12: BMI, U&Es, TFTs
Levels - minimum 3/12
Drugs causing neutropenia
Chemotherapy
Carbamazepine
Surgical VTE Prophylaxis with a needle phobia
Apixaban 2.5mg BD
or
Rivaroxiban 10mg OD
Drugs causing hyperkalaemia
I SHAT one when I saw they had hyperkalaemia
SHAT Spironolactone / Eplerenone Heparins ACEi Tacrolimus / Ciclosporin
