Intradisciplinary Health Care Team
team members work together within the same discipline; other disciplines are not involved; not the most efficient as only one discipline is involved and the P/C only receives one type of care
(ex. PT/PTA working in a home health setting where other services aren’t needed)
Interdisciplinary Health Care Team
members work together within all disciplines to set goals relevant to a P/C’s individual case; all members collaborate in decision-making, but evaluations are completed independent of one another; most efficient and successful for patient outcomes
(ex. SNF team)
Multidisciplinary Health Care Team
members work separately and independently in their different disciplines; teams do not meet or try to collaborate; allegiance is mostly geared toward their particular discipline - competition can develop
