Awake Fiberoptic Intubation
A
- In order to successfully perform an AFOI, I need to block the glossopharyngeal nerve, superior laryngeal nerve, and recurrant laryngeal nerve.
- Prior to starting, I would have the difficult airway cart in the room and a backup plan in case of failed attempts.
- After bringing the patient to the OR, attaching monitors, and starting high-flow nasal canula, I would spray 10cc of 4% lidocaine into the oropharynx with an EZ-atomizer. Next I would apply 4% topical lidocaine to the posterior tongue and tonsillar pillars with a tongue depressor. I would then spray 5cc of 4% just past the vocal cords with a curved atomizer.
- Next I would test the adequacy of block, sedate with dexmedetomidine and perform the intubatino with the patient upright and with a fiberoptic scope.
