Primary Survey/Resuscitation Flashcards
Step 1
- Verbalize general impression of the patient.
“My general impression of the patient is_________”
Step 2
- Determine responsiveness/level of consciousness.
“Patient is appears to be alert and responsive. Hello my name is Aaron can you tell me what your name is? Can you tell me where you are right now? Lastly, can you tell me what the day of week it is currently?”
“Patient does not appear to be alert. If you can hear me, can you please squeeze my hand or make a gesture to let me know you can hear me (V)? I will now give the patient a sternum rub. I will now pinch the patients nail bed to determine responsiveness to painful stimuli (P). Patient is responsive to verbal/painful stimuli OR patient is unconscious.”
Step 3
- Determines Chief Complaint/apparent life threats.
“Can you please tell me what’s currently bothering you the most?”
“Apparent life threats would be___________”
Step 4
- Assess ABC’s
A: “Based on mechanism of injury, I will assess my patients airway utilizing a jaw thrust maneuver/head tilt chin lift. Are there any obstructions in the airway?” Insert OPA if indicated. If gagging insert NPA. VERIFY HEAD INJURIES.
B: “Is my patient breathing? What is their respiratory rate? I will now auscultate lung sounds. Based on my findings I will go ahead and put my patient on high flow O2 via non-rebreather.” MANAGE ANY INJURY THAT MAY BE COMPROMISING BREATHING (i.e. sucking chest wound).
C: “I will go ahead and assess circulation. Does my patient have a radial pulse? What is the quality and rate of my patients pulse?
Assessing condition of skin, do I notice any abnormalities?” Initiate bleeding controls (i.e. direct pressure and/tourniquet application).
INITIATE SHOCK MANAGEMENT (i.e. proper position of patient and hypothermia prevention).
Step 5
- Verbalize patient priority and make treatment/transport decisions.
