PPROM - epi
A
1/3 of preterm deliveries. 1/3 of PPROM have overt infection

PPROM chorioamnionitis - background
A
May cause few symptoms but associated with significant neonatal morbidity and mortality. Also associated with significant risks to the mother

PPROM chorioamnionitis - hx
A
- Vaginal loss - gush, constant trickle or dampness (ROM)
- Fever
- Malaise
- Abdominal pain, including contractions
- Purulent/offensive vaginal discharge

PPROM chorioamnionitis - ex
A
- Maternal pyrexia
- Maternal tachycardia
- Uterine tenderness
- Fetal tachycardia
- Speculum - offensive vaginal discharge (yellow/brown). NO VE bc increased risk of infection

PPROM chorioamnionitis - ix
A
- FBE
- CRP (raised -> indicates infection)
- Swabs (high vaginal swab, low vaginal swab)
- MSU
- U/S for fetal presentation, EFW and liquor volume

PPROM - mx
A
Mx - if chorioamnionitis (3)
- Steroids (betametasone IM)
- Deliver whatever the gestation
- Broad spectrum abx cover (?)
Mx - if no chorioamnionitis (3)
- Admit. Inform SCN, liaise with neonatologists
- Steroids (betametasone IM, 2 doses 24h apart)
- Abx (erythromycin)

A
- Prematurity
- Infection
- Cord prolapse, abruption
- Pulmonary hypoplasia
- Limb contractures
