Causes of Pressure Ulcer (4)
- Pressure
- Moisture
- Shear
- Friction
Pressure
compression of soft tissue between a bony
prominence and bed
Moisture
over-hydrates skin which lowers its resistance to stresses like pressure or friction
Shear
adjacent surfaces slide across one another
Friction
skin is pulled across a coarse surface, and skin is exposed to let bacteria enter
rotational shear
caused when the user is pivoted from the
lying to sitting position
Stage 1 Pressure Ulcer
reddened area of skin that is “non-blanchable”
Stage 1 PU indicators (4)
- Discoloration of skin
- Warmth
- Edema (swelling)
- Induration (hardening of skin)
Stage 2 Pressure Ulcer
skin blisters or forms open sore and partial skin loss
Stage 3 Pressure Ulcer
skin breakdown looking like a crater
Stage 4 Pressure Ulcer
so deep that there is damage to muscle and bone
Factors for Pressure Sores (4)
- inability to move body without assistance
- chronic condition, like diabetes
- urinary/ bowel incontinence
- fragile skin with old age
Pressure relief in wheelchairs (4)
- seat cushions
- tilting/ reclining
- repositioning
- proper seat positioning
