Parts of a prescription
A. Name and address of the patient
- Date
- age is written if the patient is a child
B. Superscription - R/ = “recipe” or “take thou of”
C. Inscription - Consists of the generic name of the drug, dosage form, amount per dose and/or strength of the preparation
D. Subscription - directions to the pharmacist
E. Signatura - “label” (directions to the patient
F. Prescriber’s signature - Included is the prescriber’s address, PRC#, PTR#, and Opium License number if the prescribed drug is regulated or prohibited
A prescription for the treatment of malaria for Mia Lara, a single non-pregnant nurse, using Quinine sulfate tablet at a dose of 325 mg twice a day, for one week. (Quinine sulfate is available in 325 mg tablets)

REMEMBER
Multiple dose method is preferred over the Single Dose Method whenever prescriptions for powder or capsules containing single or multiple ingredients are made
This gives the pharmacist the exact measurements/total dosage to be taken by the patient, thus, is easier to prepare
REMEMBER
Single dose method shifts the burden to the pharmacist and is not considered the best form
It is best that the physician do the calculations himself
**In cases of very potent drugs, the single dose method is preferred
TOTAL NO. OF DOSES = Frequency of administration per day x Duration of treatment (days)
TOTAL DOSE = Usual Dose x Total No. of Doses
REMEMBER
Papers should be used if the weight of a single dose of the drug (in prescriptions with single ingredient) or the weight of the combination of drugs (in prescriptions with multiple ingredients) is 600 mg or more
If the weight of the single dose is less than 600 mg, capsules should be used, provided the patient can swallow the solid dosage form
Justin Bibera, a 10 year old, 30 kg boy from Beverly Hills, Taytay with typhoid fever, requires a prescription for chloramphenicol, using the powder form, to be given at 100 mg/kg/day in 4 divided doses, round the clock, for 10 days. Use MULTIPLE DOSE METHOD.

- *The computation by multiple dose is as follows Total no. of doses = frequency of administration x duration of treatment: 4 x 10 = 40
- *Total dose = usual dose x total no. of doses: 30 kg x 100 mg/kg/day x 10 days = 30 g
A prescription for Bhoy, a 5-year old boy from San Juan weighing 15 kg, who was diagnosed to have pharyngitis to be given Amoxicillin in powder form, of a dose of 50 mg/kg/day in 3 divided doses round the clock for 7 days. (Use SINGLE METHOD)

- *Dose: 15 kg x 50 mg/kg/day = 750 mg of Amoxicillin is required per day
- *Total dose: Dose/Number of preparation = 250mg
- *Technically, since the computed weight of the single dose is less than 600 mg (250 mg), the powdered drug should be placed in capsules. But since the patient is a 5-year old child, he may have difficulty in swallowing the capsule, so we dispense it in papers instead.
A prescription for Ellen Arce, a 30 yo female with constipation using Bisacodyl for 10 mg suppository once a day at bedtime for 2 doses. Available on 10 mg suppositories.

- *Always indicate the DRUG ROUTE. Otherwise, the patient would not know how to use the drug. She might mistake it as a drug taken orally.
- *Do not forget to indicate (and emphasize) the specific time of the day in which the drug shall be taken (in this example, at bedtime).
- *Assume that the suppository will be used for 2 consecutive days since it is not specifically indicated
