Non-Controlled Substances
Not recognized by federal law as being an abused drug when used for non-medical purposes or used in excessive quantities
Schedule I (3)
- High potential for abuse
- No currently accepted medical use
- Lack of accepted safety adata
(ex: Heroin)
Schedule II (3)
- High potential for abuse
- Currently accepted medical uses
- Abuse of drug may lead to severe physical and/or psychological dependence
(ex: ADHD medications, opioids)
Schedule III (3)
- Less potential for abuse than schedule II
- Currently accepted medical uses
- Abuse may lead to moderate/low physical dependence or high psychological dependence
Ex: Fioricet vs. Firoinal
Schedule IV (3)
- Less abuse potential than Schedule I,II,III
- Currently accepted medical uses
- Abuse may lead to limited physical or psychological dependence
Schedule V (4)
- Less abuse potential than I-IV
- Currently accepted medical uses
- Abuse may lead to limited physical or psychological dependence

- Diazepam

- Buprenorphine (narcotic for pain)

- NYS seal fades from blue to clear
- “Standard register” turns orange from yellow
- ONLY ONE MEDICATION per prescription blank is allowed in NYS regardless of the type of Rx blank

- Telephone Number
- License number/state registration/certificate number
- Professional designation
Patient Information
- Name
- Complete Address
- Age or DOB
Non-Controlled Rx Rules (7)
- Valid for 1 year from date written (except Medicaid is valid for 6 months)
- Need inscription of drug name, strength, and dosage form
- Need subscription of directions for pharmacist and quantity to dispense
- Indicate number of refills
- All prescriptions are generically filled unless DAW is filled in appropriate box
- All medications must be signed handwritten in ink
- Some medications require a diagnosis on face of the Rx
Non-Controlled Rx Medicaid Rules (3)
- Valid for 6 months from date written
- Maximum of 5 refills w/i 6 months and can only be refilled when 10 day supply is left
- If it is not going to be generically filled there must be “brand medically necessary” written
Controlled Substance Prescriber Information (7)
Heading should contain:
- Name
- Address
- Telephone number
- DEA registration license number
- State registration number
- Profession
- Institution ONYSRX: institution name, address, DEA
Controlled Substance Patient information (5)
- Full name
- Complete address
- Age or DOB
- Gender (ONYSRx)
- Condition code (for over 30 day supply)
