Prescribing in Palliative Care Flashcards
What is licensed for treatment of intractable hiccups in palliative care? (2)
1) Chlorpromazine
2) Haloperidol
2 key steps in conservative mx of secretions in end of life?
1) Avoiding fluid overload - stopping IV or SC fluids
2) Educating the family that the patient is likely not troubled by secretions
1st line mx of secretions in palliative care?
Hyoscine hydrobromide or hyoscine butylbromide
Glycopyrronium bromide may also be used
What are 6 broad N&V syndromes in palliaite care?
1) Reduced gastric motility
2) Chemical
3) Visceral/serosal
4) Raised ICP
5) Vestibular
6) Cortical
What is chemically mediated N&V in palliative care typically 2ary to? (3)
1) Hypercalcaemia
2) Opioids
3) Chemotherapy
What 2 receptors is reduced gastric motility causing N&V related to?
1) Serotonin
2) Dopamine
What is raised ICP causing N&V in palliative care usually in context of?
Brain mets
1st line mx of N&V due to reduced gastric motility?
Pro-kinetic agents –> Metoclopramide & domperidone
Note - should NOT be used in complete bowel obstruction, GI perforation, or immediately following gastric surgery.
Mx of chemically mediated N&V?
1) Correct chemical disturbance (if possible)
2) Mx –> haloperidol, levomepromazine, ondansetron
What 3 medications may be used in mx of chemically mediated N&V?
1) haloperidol
2) ondansetron
3) levomepromazine
2 causes of visceral/serosal N&V?
1) Constipation
2) Oral candidiasis
1st line in mx of visceral/serosal N&V? (2)
Cyclizine & levomepromazine
Mx of raised ICP causing N&V?
1) Cyclizine
2) Dex can also be used
1st line mx of vestibular causes of N&V?
Cyclizine
1st line mx of agitation in TERMINAL phase?
Midazolam
