A survey questionnaire can provide accurate and relevant data through thoughtful design, testing, and detailed administration (McClelland, 1994).
Questions asked will only require a simple “yes” or “no” answer or an “agree” or “disagree” response. However, this type of questions must be combined with other types since they may not give the researchers the best kind of data to be analyzed.

These questions provide respondents options wherein they may be asked for a single response or multiple answers.
MULTIPLE CHOICE QUESTIONS
The respondents are asked to express their opinions, insights, and explanations about certain matters. Multiple answers and varied lengths are expected in this type of questioning.
OPEN-ENDED QUESTIONS
This type of question asks the respondents to rank form highest to lowest or lowest to highest or least important to most important or vice versa the items provided. The answers will reveal discriminations or preferences.
RANK ORDER SCALE QUESTIONS
