Pre-Op Complications Flashcards
What comprises of VTE?
DVT
PE
Why is VTE screening and prophylaxis important?
60% occur following admission to hospital
No 1 cause of preventable death in hospitals
What is Virchows triad?
Venous stasis
Abnormal coagulation
Vascular intimal damage
What two things are considered in VTE risk screening?
Patient factors
Admission factors
What two things need to be balanced in risks assessment of pt?
Thrombotic risk vs bleeding risk
What are the pharmacological pre-operative prevention options for VTE?
LMWH, UFH
Warfarin, DOACS, antiplatelets
What are the non-pharmacological preoperative options for VTE?
Anti-embolism stockings
Intermittent pneumatic compression (dynamic)
What therapy may pts of warfarin have peri-operative?
Bridging therapy by switching to LMWH
What preoperative factors increase the risk of post operative DVT?
Immobilisation Obesity Smoking Alcohol FH Increasing age Pregnancy Cancer Past DVT Thrombophila Synthetic oestrogen - COCP
What intraoperative factors increase risk of DVT?
GA
Trauma
Orthopaedic surgery
How do graduated compression stocking reduce risk of DVT?
Prevent formation of blood clots by applying varying amounts of pressure in different parts of the leg. This increase the venous return/reduces the peripheral venous pooling.
