Ask about DVT, PE and other clotting or bleeding problems.
History of allergies, especially to anaesthetics

Social support, living situation, occupation, mobility, any dependents

Review of systems including: Last menstrual period and pregnancy Tetanus immunisation Dentures and crowns GI tract: dysphagia, GORD, anorexia or weight loss, change in bowel habit, rectal bleeding CVS/Resp: chest pain, shortness of breath, cough, sputum, palpitations Urinary symptoms Fits, faints, funny turns Rashes or joint pain
