RIBS – AP UPPER to demonstrate the RIGHT or LEFT side

A

Posture: Back of hands on hips and shoulders rolled forward , lift chin

CP: Midway between the MSP and the outer margin of the thorax, 4” below jugular notch

Collimation: Includes C7, open all the way down, light from spine to lateral margin

SID: 180

Breathing: Deep inspiration

Notes: Hands out of light

CHEST – STRETCHER AP SEMI ERECT

A

Posture: Back of hands on hips and shoulders rolled forward, lift chin, push pt. right against IR

CR: Match to sternum angle or IR angle -5°

CP: MSP, 4” below the jugular notch, Line laser up with center of IR

Collimation: Includes C7 (2” above shoulders), open to length of IR, light to lateral margin

Breathing: Suspend after 2nd full inspiration

SID: Max SID achievable (bring tube up all the way, then move away until centered)

Notes: arrow sideways, no grid needed, ensure light isn’t off IR

A

Posture: Erect

CP: MSP at level of xiphoid

Collimation: light to side of soft tissue, 4” above and below center

Breathing: Expiration

CHEST – DECUBITUS on stretcher to demonstrate hemothorax in RIGHT or LEFT lung

A

Posture: PA or AP, Lying on side, bent legs, arms above head, Bucky right against pt., chin out of light, Right side up for hemothorax in left, Left side up for hemothorax in right

CP: Place the pts. shoulder 2” medial to bucky, AP: MSP and 4” below jugular notch, PA: midline at level of T7

Collimation: C7 in light open all the way down , light on stretcher and on Bucky

Breathing: Suspend after 2nd full inspiration

SID: 180cm

Notes: Leave pt. in position for 5 min, arrow up, pillow out of light, line up Bucky and tube then pump up stretcher, watch rails, white part of IR at foot end

STERNUM – LATERAL

A

Posture: Left, no leaning (spine parallel to IR), hands behind back and shoulders REALLY rolled back (chest stuck out)

CP: Midway between jugular notch and xiphoid

Collimation: 1” above jugular notch and 1” below xiphoid (center AP first), angle collimator to match sternum

SID: 180cm

Breathing: Deep inspiration

STERNOCLAVICULAR JOINTS – PA Oblique to demonstrate the RIGHT or LEFT side

A

Posture: RAO 30°- 40° demos right, LAO 30°- 40° demos left, ensure jugular notch in front of spine

CP: 2” lateral to MSP on raised side at level of Jugular notch (center with pt. AP first), CR going through SC joint

Collimation: light to spine, small square

SID: 100cm

Breathing: Suspend respiration

Notes: Use R marker only when demos right, Use L marker only when demos left

A

Posture: Back of hands on hips and shoulders rolled forward (touching Bucky), lift chin

CP: MSP at T7 (apex of scapula)

Collimation: Includes C7 (2” above shoulders), open all the way down, light to lateral margin

A

Posture: AML (line from acanthion to EAM) perp to the IR

CP: 1” above jugular notch

Collimation: Top of light at the symphysis menti (tip of chin), light to Carina (armpit), lateral to soft neck tissue

Breathing: Slow deep inspiration

SID: 100cm

A

Posture: Supine, arms at sides out of light

CP: MSP between T8 (2” above xiphoid) and iliac crest

Collimation: 2” above xiphoid process to iliac crest, light to lateral borders (open all the way)

A

Posture: Bent legs, PA or AP, arms above head, Bucky right against pt.

CP: MSP, 2” superior to the iliac crest

Collimation: 2” above xiphoid (line up xiphoid with bucky), open light all the way up and down, light on stretcher and bucky

Breathing: Expiration

SID: 100cm

Notes: Leave pt. in position for 5 mins, arrow up

STERNUM – OBLIQUE

A

Posture: RAO 30°- 40° (move sternum off spine)

CP: 2” left of the midline (on raised side) and midway between jugular notch and xiphoid (center with pt. AP first)

Collimation: 1” above jugular notch and 1” below xiphoid, right edge of light to spine

SID: 100cm

Breathing: Shallow breathing

CHEST – AP LORDOTIC

A

Posture: Sit or Stand the patient 1 ft from the bucky, lean back and rest shoulders against bucky, roll shoulder forward, spine straight

CP: Top of IR 3” above shoulders, mid sternum

Collimation: C7 in light open down to above crest (collimate), light to lateral margins

Breathing: 2nd full inspiration

SID: 180cm

SOFT TISSUE NECK – LATERAL FOR UPPER AIRWAY

A

Posture: AML (line from acanthion to EAM) parallel to the floor

CP: level of C5 (thyroid cartilage), where chin and neck meet

Collimation: Light from nasion (just below eyebrow) to jugular notch, entire nasal cavity, not all of neck

Breathing: Slow deep inspiration

SID: 180cm

A

Posture: Left, no leaning- hip and shoulder not touching at same time (MSP parallel to IR), arms raised above head and forward holding pole

CP: MCP, 1” below T7 (apex of scapula)

Collimation: Include anterior posterior aspects, Open all the way up and down

Breathing: 2nd full inspiration

SID: 180cm

Notes: Lower Bucky 1” from height used with PA, C7 not on

CP: MSP, 2” superior to pubic symphysis (greater trochanter)

Collimation: ASIS to pubic symphysis, Side to side to ASIS

A

Posture: RAO 45° demos left side, LAO 45° demos right side, raise arm of affected side and place on bucky

CP: Level of T7 (apex of scapula), midway between lateral margin and spine

Collimation: Includes C7, lateral portion of ribs, light from spine to lateral borders

Breathing: Deep inspiration

SID: 180cm

CHEST – DECUBITUS on table to demonstrate pneumothorax in RIGHT or LEFT lung

A

Posture: PA or AP, Lying on side, bent legs, arms above head, IR right against pt., chin out of light, Right side up for pneumothorax in right, Left side up for pneumothorax in left

CP: Place the IR 2” above shoulders and move light to top of IR, AP: MSP and 4” below jugular notch, PA: midline at level of T7

Collimation: C7 in light open all the way down , light on mat and on IR

Breathing: Suspend after 2nd full inspiration

SID: 180cm (as far as you can get), move pt. to back of table and move table back

Notes: Leave pt. in position for 5 min, arrow up, pillow out of light, white part of IR at foot end

Posture: Back of hands on hips and shoulders rolled forward, lift chin CR: 5° caudad CP: MSP, 4" below the jugular notch Collimation: Includes C7 (2" above shoulders), open all the way down, light to lateral margin Breathing: Suspend after 2nd full inspiration SID: Max SID achievable (table all the way down and tube all the way up) Notes: arrow down, pillow out of light, watch for diverging beam

CP: MSP at level of iliac crest Collimation: superior aspect of the symphysis pubis (feel for greater trochanter- more important to get on than centering), open up and down all the way, lateral borders of abdomen Breathing: Suspend on expiration SID: 100cm Notes: For KUB collimate side to side to ASIS, arrow down, no shielding
