Priority is ensuring patients are not in immediate danger, speaking with someone who can intervene and act

Individual - check in with colleagues if any doubts, be aware of switch points and question decision, reflect on previous decisions / outcomes, identify areas for improvement
Organisational / systemic - ensure properly staffed with shifts/breaks often enough to ensure people are not overworked, regular feedback with peers / senior leaders, review meetings following error
