Illness/Injury Leave Report:
1.1. Used by the Department to track all sick and injury leave.
1.2. Member’s responsibility to complete any time that illness/injury leave is used.
1.3. Must be completed whether or not the member receives treatment.
1.4. Shall be returned to the Spokane Fire Department Human Resource Officer (HRO) in
the first mail run following return to duty.
1.5. Applies to LEOFF l, LEOFF II and Civilian Dispatch.

Employee Injury Report:
2.1. Used by the Department and City Risk Management to track all on duty injuries for the
City and Labor & Industries.
2.2. Should be completed before going off shift or within twenty four (24) hours of the
reported injury. An explanation of the nature and extent of injury shall be included.
2.3. The report will be forwarded immediately to the Spokane Fire Department HRO
2.4. Applies to LEOFF l, LEOFF II and Civilian Dispatch

Physician’s Disability Statement (PDS):
3.1. Used by the Department and Risk Management to determine sick/injury status and to
document an L & I claim.
3.2. Applies to LEOFF l, LEOFF ll and Civilian Dispatch members, who are absent from
work and seek medical treatment.
3.3. Shall be submitted to the physician at the first appointment.3.4. Should be completed by the attending physician during the appointment time and
returned to the member upon completion of the appointment.
3.5. The member will forward the PDS original and second copy (yellow) by mail or hand
delivery to the Spokane Fire Department HRO in the self addressed envelope. The
PDS original copy may be faxed to the Spokane Fire Department HRO in lieu of mailing
or hand delivery. The Spokane Fire Department fax number is 625-7039.
3.6. The PDS shall be mailed, delivered or faxed by the member to the Spokane Fire
Department HRO on the day of completion by the physician.

Self-Insurer Accident Form (SIF-2):
4.1. Used by City Risk Management and Labor & Industries.
4.2. Applies to LEOFF ll and Civilian Dispatch employees who seek medical treatment for
injury on duty.
4.3. Completed by the injured employee and forwarded to the Spokane Fire Department
HRO within 24 hours of the reported injury.

apparatus. The packets contain the following forms/reports:

- 10 Anticipated return to duty date
