General clinical features?
Fever, productive cough, SOB, pleuritic chest pain
raised inflammatory markers, new CXR infiltrate
How to assesse severity for CAP
CURB65 C-Confusion U-Urea>7 R-Respiratory rate>30 B-BP<90/60 65-age>65
Associated with high fever, rapid onset, herpes. increasing age and co-morbidity
Strep Pneumonia
Common in patients with COPD
H.influnzae (can also cause epiglottitis)
younger patients and after travel
More severe infection, evidence of a multisystem disease
Legionella
Younger patient who has had prior antibiotics and a dry cough with haemoptysis , atypical CXR findings and flue like symptoms
Mycoplasma pneumonia
Recent influenza illness
Staph aureus
In alcoholics, elderly, diabetics.
have low platelet count
Klebscella pneumonia
In HIV pateints
Pneumocystis jiroveci
Long duration of symptoms
Headache, fever, dry cough, SOB
Chlamydia Psttaci
Is caught from birds and animals
Chlamydia Pneumonia
Pharyngitis, hoarseness, otitis followed by pneumonia
Treatment for Sterp Pneumonia
Amoxicillin IV
For H. influenza
If non-lactamase producing Amoxicillin PO
If lactamase producing Co-amoxiclav IV/PO
For legionella
Clarithromycin PO/IV
For Mycoplasma pneumonia
Clarithromycin or fluroquionolone
