What is plural effusion
When fluid accumulates in pleural space
Aetiology of pleural effusion
Transudate - Low protein content <30g/L
Exudate - High protein content >30g/L
What causes transudate
Disturbances in oncotic pressure
- Cardiac failure
- Hypoproteinaemia
- Liver cirrhosis
What causes exudate
Fluids that left circulatory system go into lesions/areas of inflammation, commonly due to infection or malignancy
- Pneumonia
- TB
- Connective tissue disease
- Asbestos
Symptoms & signs of pleural effusion
Chest pain
Dry cough
SOB
Difficulty taking deep breath
Reduced chest expansion (affected side)
Stony dull percussion
Investigations of pleural effusion
CXR
Aspiration of fluid (thoracentesis)
If diagnosis not clear from above = Pleural biopsy
How much fluid needs to be present to be visible on CXR
> 500ml will cause a clear fluid level
What is assessed in aspiration sample
Colour
Cytology
Microbiology
pH
Glucose
What does straw coloured fluid indicate
Cardiac failure
Hypoalbuminemia
What does bloody fluid indicate
Trauma
Malignancy
Infection
Infarction
What does milky/turbid coloured fluid indicate
Empyema
Chylothorax
What does foul smelling fluid indicate
anaerobic empyema(pus)
What does fluid with food particles indicate
Oesophageal rupture
What does bilateral fluid indicate
LVF
PTE
Drugs
Systemic path e.g. lupus
What can cytology indicate
Lymphocytes indicate malignancy or TB
Neutrophils indicate an infection
