Plantar Fasciotomy Flashcards
what is the etiology of plantar fasciitis?
- repetitive traction w/ inflammation
- pronation
- increased body weight or activity
- systemic or rheumatological disorders
what are some DDx of heel pain?
- bursitis
- contusion/ stress fx
- tendonitis
- nerve entrapment
- neuroma
- systemic dz
- infection
what is post-static dyskinesia?
pain that first arises in the morning or when you get up from a seated position
what are some conservative tx options for heel pain?
taping, NSAIDs, ice , PT, orthotics, injections, night splints, immobilization, platelet-rich plasma injections
what are the 3 types of plantar fascial release procedures?
- instep fasciotomy
- open plantar fasciotomy
- endoscopic plantar fasciotomy
what is the only plantar fascial release procedure that allows for spur resection?
open plantar fasciotomy
describe the instep fasciotomy procedure.
- incision in medial arch anterior to calcaneus
2. medial portion of fascia is released
describe the advantages/disadvantages of instep fasciotomy.
- good recovery and visibility
- but unable to resect heel spur
describe the open plantar fasciotomy procedure.
- medial incision is made to visualize the plantar fascia
2. fascia is isolated superior and inferior
what are the advantages/disadvantages of open plantar fasciotomy?
- good visibility and you can resect heel spur!
* disadvantages: longer recovery period and wound complications/ hematoma
describe the endoscopic plantar fasciotomy procedure.
use of an endoscope to visualize and cut the plantar fascia
what are the advtgs/disadvtgs of EPF.
- good recovery and visibility
- but cannot resect heel spur
what is a potential complication of resecting the heel spur?
creating a stress riser in calcaneus
